Policy for Late Submission of Self Reporting Forms for CCA Cycles
The Continuing Competency Assessment (CCA) Section and Education Department now offer a mechanism for CNMs and CMs who have participated in past CCA Program cycles but did not receive a Certificate of Satisfactory Completion either because they did not submit their Self Reporting Form (SRF) by the October 1st deadline or previous CCA Program completion options did not allow them to meet the program objectives.
What does this mean for you? If you were enrolled in a previous CCA cycle but did not receive a certificate of satisfactory completion, you may now be eligible to receive one if:
you did not receive your certificate because you did not submit your SRF by the October 1st deadline, you may complete a SRF and send it to the ACNM Education Department for processing along with a $25 late fee payable to the ACNM.
you did not receive your certificate because at the time your cycle(s) ended you had not met the program?s requirements, but under current program requirements (the expansion of alternatives in lieu of CEUs, reciprocity for Accreditation Council for Continuing Medical Education (ACCME) Category I activities, and/or removal of the requirement for completing 10 hours each in designated areas) you would be eligible, you may now complete a current SRF and send it to the ACNM Education department for processing. (You must have submitted a SRF by the reporting deadline for the cycle in order to be eligible for this option. If you did not submit a SRF, see the option above.)
In both of these cases, you may take advantage of the current CCA Program options for demonstrating satisfactory completion of the CCA cycle, HOWEVER, the 50 contact hours/5 CEUs must have been received within the dates for the CCA cycle in which you were enrolled. These options are listed on current SRFs. The CCA cycles and dates inclusive are:

All SRFs submitted under this option may be subject to audit by the CCA Section. If your SRF is selected for audit, you will be required to submit copies of documentation verifying you completed the options listed on your SRF. Certificates of Satisfactory Completion will be sent out 60 days after receipt of your SRF, unless you are selected for audit, in which case it will take 90 days.
You must have already been enrolled in the CCA cycle in order to receive a certificate of satisfactory completion. This option does not allow you to retroactively enroll in a previous cycle to receive a certificate of satisfactory completion. The ACNM Board of Directors approved the option of accepting late submissions of SRFs at their quarterly meeting in November 1997.
